The South Shore Child Guidance Center Aftercare Program is dedicated to providing follow up services to children who are discharged from inpatient psychiatric units. The overall goal of the program is to provide short-term intensive home based services to these children and their families to reduce the risk of re-hospitalization. Aftercare services will provide essential outreach, intervention, and coordination with other community agencies in order to augment the Home Based Crisis Intervention Program.

Eligibility, accessibility, services and goals are similar to those for HBCI. There are no fees for the Aftercare Program. Funding is provided by the Nassau County Department of Mental Health, Chemical Dependency and Developmental Disabilities and New York State Office of Mental Health.
